South Shore Support Services assists 32 people in a wide variety of living situations through individual supports.
In many cases the services of by South Shore Support Services allow these people the level of assistance they need to maintain their independence in the community.
A case manager is assigned to each individual and works with the individual to create a plan for addressing areas of need. If an individual needs a great deal of support they may be assigned more than one case manager.
Individuals in the individual support program work on skills including but not limited to banking/finances, medication administration/review, nutrition/healthy living, transportation, medical appointments, general housekeeping and socialization.
The individuals are allotted a specific number of case management hours per week based on the decision of the Department of Developmental Services.
