Adult Family Care

Quality In-Home Care for Adults with Disabilities

Remaining at home in a familiar, caring environment is essential to maintaining independence, dignity, and quality of life. Many individuals with medical, physical, or developmental disabilities rely on the daily care provided by dedicated family members or friends.

The Adult Family Care (AFC) program at South Shore Support Services provides the formal framework, nursing oversight, training, and financial compensation (tax-free stipend) that make high-quality, home-based care sustainable for families across our community.

Participant Eligibility Requirements

The Intake Process:

  • Reach Out to Our Team

    Contact Director of AFC Katie Stronach at kstronach@soshoresupport.org or call our office at (781) 331-7870. We will conduct a brief phone interview to review basic insurance and care needs.

  • In-Home Evaluation & Care Plan

    Assessment by Nurse & Case Manager. Our intake team schedules a visit to your home to conduct a clinical assessment, evaluate eligibility, discuss daily routines, and outline your customized care plan.

  • MassHealth Authorization & Caregiver Orientation

    Approval & Setup. We submit all clinical documentation to MassHealth for stipend level determination and program authorization. Once approved, we guide the caregiver through orientation and training.

  • Ongoing Monthly Support

    4.Ongoing Monthly Support:Home Visits & Stipend Distribution.Caregivers begin receiving their monthly tax-free stipend. Our RN and Case Manager conduct regular home visits and phone consultations to ensure continuous support and clinical oversight.

Eligible Caregivers Include:

  • Non-custodial parents (for adult children age 16+)

  • Adult children, siblings, aunts, uncles, or extended family members

  • Friends or qualified non-family members living in the home

Ineligible Caregivers:

Under MassHealth regulations, caregivers cannot be:

  • Spouses

  • Parents of minor children (under age 16)

  • Legally responsible relatives (such as legal guardians)

Katie Stronach

Katie is a seasoned human services professional with over 20 years of experience, including 15 years in leadership, supported by a strong record of CARF accreditation and regulatory compliance across multiple funding sources.

Katie attributes her success to exceptional communication skills, advanced management training, and her ability to build strong relationships with clients, multidisciplinary teams, and key stakeholders.
